Curious about what happens when you need to cancel your session?
We wanted to take a moment to reiterate our Cancellation Policy as we have had an increase in No Shows & Late Cancellations the past couple of months.
The FORM Collective is an appointment based business, when you book a service with us that time slot is held exclusively for you. As a Collective of individual Practitioners, we do not have a receptionist who can try to fill your session while we are hands-on in treatment. Therefore, a cancellation with a short notice is a missed opportunity for both our Practitioners and other Guests.
We realize that most of our Guests are on time and follow appointment etiquette and we hate to have to enforce a Cancellation Policy. Please remember this policy is not in place to “take your money” or to upset anyone. These policies help create professional boundaries and support the therapeutic relationship between the Practitioner and Guest.
To do our part, the clinic sends confirmation texts and/or emails confirming your session date and time after booking and again prior to your appointment giving you plenty of time to cancel or reschedule should you need to.
LATE ARRIVAL POLICY
As a courtesy, our Late Arrival Policy includes a 15 minute grace period. We understand traffic, unexpected weather, and life happens. However as we are booked back to back we cannot accommodate extending your session, therefore if you are later than 15 minutes to your appointment, your appointment will automatically be considered a No Show and will be charged as such.
NO SHOW/ LATE CANCELLATION POLICY
Any No Shows or Cancellations less than 48 HOURS PRIOR to your appointment time will result in a FULL CHARGE of the appointment amount booked to your credit card on file on the session date.
This Cancellation Policy is stated in our consent forms that must be agreed upon and before booking your session. We hold everyone equally accountable. Failure to pay will result in your scheduling access to be restricted indefinitely.
